The sponsor of this Sweepstakes is Wynden Stark LLC dba GQR Healthcare ("Sponsor"). By participating in the Sweepstakes, each Entrant unconditionally accepts and agrees to comply with and abide by these Official Rules and the decisions of Sponsor, which shall be final and binding in all respects. Sponsor is responsible for the collection, submission or processing of Entries and the overall administration of the Sweepstakes. 2.   ELIGIBILITY: Open to candidates who are on a healthcare assignment with GQR from November 1^st, 2022 through December 8^th, 2022, and who are legal residents of any state in the United States excluding U.S. territories, at least 18 years of age, and able to provide the necessary information to claim the prize. Sponsor, and their respective parents, subsidiaries, affiliates, distributors, retailers, sales representatives, advertising and promotion agencies and each of their respective officers, directors and employees (the ”Promotion Entities”), are ineligible to enter the Sweepstakes or win a prize. Household Members and Immediate Family Members of such individuals are also not eligible to enter or win. "Household Members" shall mean those people who share the same residence at least three months a year. "Immediate Family Members" shall mean parents, step-parents, legal guardians, children, step-children, siblings, step-siblings, or spouses. 

By participating, Entrant agrees to be fully unconditionally bound by these Official Rules, and Entrant represents and warrants that you meet the eligibility requirements.

This Sweepstakes is subject to all applicable federal, state, and local laws and regulations and is void where prohibited or restricted by law. 

3. PRIZES: Grand Prize: 12 winners will receive a $100 gift card to Door Dash. 

12 prizes will be awarded. Prizes cannot be transferred or substituted by winner. Sponsor reserves the right in its sole and absolute discretion to award a substitute prize of equal or greater value if a prize described in these Official Rules is unavailable or cannot be awarded, in whole or in part, for any reason. The ARV of the prize represents Sponsor's good faith determination. That determination is final and binding and cannot be appealed. If the actual value of the prize turns out to be less than the stated ARV, the difference will not be awarded in cash. 4. HOW TO ENTER: Current candidates who are contractors of Sponsor during the Promotional Period are automatically provided 1 Entry into the Sweepstakes. An Entry must fulfill all sweepstakes requirements, as specified, to be eligible to win a prize. Entries that are not complete or do not adhere to the rules or specifications may be disqualified at the sole discretion of Sponsor.

Automated or robotic Entries through the website that are submitted by individuals or organizations will be disqualified. Internet entry must be made by the Entrant. 5. WINNER SELECTION: The Winner of the Sweepstakes will be selected in a random drawing from among all eligible Entries received throughout the Promotion Period.

The random drawing will be conducted December 9^th, 2022, December 16, 2022, December 23, 2022. Winners 1 through 4 will be drawn December 9^th, 2022 and notified by email and phone approximately 4 hours after drawing.  Winners 5 through 8 will be drawn December 9^th, 2022 and notified by email and phone approximately 4 hours after drawing.  Winners 9 through 12 will be drawn December 9^th, 2022 and notified by email and phone approximately 4 hours after drawing.

GQR Healthcare First Annual Photo Contest

Term:

GQR Healthcare’s First Annual Photo Contest begins November 28, 2022, and ends December 2, 2022, at 5:00 PM Eastern Standard Time. By submitting an entry, each contestant agrees to the rules of the contest and states that he or she is 18 years old or older.

Who may enter: GQR Nursing & Allied who are currently on assignment with GQR. 

Individuals affiliated with the GQR Healthcare, including employees, trustees, interns, volunteers, fellows, research associates and their immediate families (children, parents, siblings and spouses) and others living in their households are not eligible for any prize. GQR will determine winners’ eligibility in its sole discretion.

What to enter:

To be eligible, a photograph must have been shot by the entrant since January 1, 2019[GU1] . This date applies to all images in a composite. 

The contestant certifies that he/she is the sole creator and copyright owner of the submitted photograph. In addition, the contestant must agree to the following:

I own the rights in the photo I uploaded (the "Work") that is depicted in a photograph or photographs taken by me. 

I give GQR Healthcare permission to use my Work. I give the GQR Healthcare (and those authorized by GQR Healthcare) a royalty-free, world-wide, perpetual, nonexclusive license to publicly display, distribute, reproduce and create derivative works of that entry, including the depiction of my Work, in whole or in part, in any media now existing or later developed, for any purpose, including, but not limited to, advertising and promotion of the magazine and its website, exhibition, and commercial products, including but not limited to GQR Healthcare publications. I understand that I shall not receive any compensation for these uses and that GQR Healthcare is under no obligation to use the entry in any way. I waive any claim or right of action arising out of the uses authorized in this release.  

For a photo in which a person is recognizable, you must be prepared to provide a model release from the subject or, in the case of a minor, the subject’s parent or guardian, to GQR Healthcare upon request. 

Similarly, entrants whose photos depict other people’s work (such as sculptures, statues, paintings, and other copyrightable works) must be prepared to provide a release from the rights holder to GQR Healthcare upon request. When photographing the work of others, it must be as an object in its environment and not a full-frame close-up of another person's creation. 

If the entrant is unable to provide all required releases, GQR in its sole discretion reserves the right to disqualify the submission, seek to secure the required releases, or permit the submission to remain in the contest.

Photos must be taken in accordance with federal, state and local laws.

Photos that violate or infringe upon another person's rights, including but not limited to copyright, are not eligible.

Photos that contain sexually explicit, nude, obscene, violent or other objectionable or inappropriate content, as determined by the GQR Healthcare in its sole discretion, are ineligible for all categories of this contest.

We do not accept photographs submitted through the mail and do not accept more than one contestant per e-mail address. High-quality scans of non-digital photographs are acceptable. Digital photographs should be taken at the highest resolution possible.

Finalists must be prepared to submit an original, unedited digital file or an original negative, print or slide. Original negatives, prints or slides will be returned to finalists. GQR Healthcare reserves the right to disqualify incomplete entries and/or contestants who are unable to submit, upon request, a high-resolution photograph of at least 300 dpi at 3,000 pixels on the longest side or an original photo negative, print or slide. Mobile photographs are exempt from this size requirement but must be submitted at their highest resolution upon request.

Conditions of Entry:

All entrants hold the Sponsors and their respective regents, directors, officers, employees, emeriti, fellows, interns, research associates, and volunteers (the “Indemnified Parties”) harmless from and against all claims of any nature arising in connection with entrant’s participation in the contest and acceptance or use of a prize. The Indemnified Parties are not liable for any costs, damages, injuries, or other claims incurred because of entrants’ participation in the contest or winner’s acceptance and usage of a prize. The Indemnified Parties are not responsible for incomplete or misdirected entries, technical or network malfunctions or failures, or causes beyond their control. Entrants are solely responsible for their entries. Entrants may not submit materials that introduce any software viruses, worms or other programs designed to damage software, hardware or telecommunications equipment or are off-topic, partisan-political, contain advertising, nudity, personal attacks or expletives, or is otherwise abusive, threatening, unlawful, harassing, discriminatory, libelous, obscene, false, sexually explicit, or that infringes on the rights of any third party.

The contest is void where prohibited or restricted by law. GQR Healthcare reserves the right to cancel the contest or modify these rules at its discretion. In the event of a dispute regarding the winners, GQR Healthcare reserves the right to award or not award the prizes in its sole discretion. GQR Healthcare reserves the right to disqualify any entrant whose entry or conduct appears in any way to: inhibit the enjoyment of others; tamper with the competition; violate these rules or other applicable law or regulation; infringe on the rights of third parties; or act in an unsportsmanlike or disruptive manner. 

Decisions of GQR Healthcare are final and binding.
